A medical school transcript for IMGs in ERAS 2027 is the school record showing subjects, study duration, and examination performance. It may be called a mark sheet, time sheet, or hour sheet.

If your school participates in ERAS Support Services through the MyIntealth Entity Portal, the school must upload it. If it does not participate and you hold an official student copy, upload the complete record as one PDF through the MyIntealth Applicant Portal.

Direct submissions take up to five business days. ECFMG recommends that MSPEs and medical school transcripts be uploaded no later than September 11, 2026, for availability when programs begin access on September 23.

A transcript previously used for ECFMG Certification does not move automatically to MyERAS. A last-resort transfer request can take up to eight business days and is unavailable when the school participates in the Entity Portal.

Residency applicants do not manually assign the transcript; MyERAS automatically sends the available document to all applied programs.

What counts as a medical school transcript?

International schools use different terms and formats. The decisive question is whether the official school record shows what was studied, how long it was studied, and how the applicant performed.

The transcript must include the applicant's full name. It is not the diploma, degree certificate, MSPE, USMLE transcript, or ECFMG Status Report.

Five records that applicants confuse

Putting the wrong document in the transcript slot delays quality control and can leave programs without the record they require.

A diploma proves award of a degree but does not show the course-by-course academic history expected from a transcript.

The upload owner decision

Ask the Dean's or Registrar's office specifically whether the school participates in ERAS Support Services through the MyIntealth Entity Portal.

A participating school must upload. The applicant cannot switch to self-upload or Certification-record transfer merely because the school is slow.

The MyERAS provider question

IMG residency applicants must indicate whether they or their school will provide the medical school transcript to ERAS Support Services at ECFMG.

A prior response appears for review and can be changed before certification. An unanswered question must be completed before the applicant certifies and submits.

This response declares the route; it does not upload, process, or deliver the document. Continue monitoring the actual status.

Applicant upload through MyIntealth

ECFMG accepts official student copies when applicant upload is allowed. It does not authorize an applicant-created academic record.

The public page directs applicants to the live MyIntealth screen for technical dimensions and file size. Use that current specification rather than an old third-party number.

Busy security paper can make a valid record unusable

ECFMG warns that a transcript should not be on security paper with a busy background because the scanned text can become illegible. It advises asking the school to copy the transcript onto plain paper.

Do not digitally erase a watermark or background yourself. Ask the school for a legible authorized reproduction.

Inspect every grade, date, decimal, course title, stamp, and signature at normal zoom before upload. Compression that hides a single mark can change how the record is interpreted.

School upload through the Entity Portal

The student's first request to the Registrar is not the start of ECFMG's five-day clock. Processing begins after ECFMG receives the submitted document.

The eight-business-day transfer timeline

A transfer can fail if ECFMG cannot fulfill the request. ECFMG sends that outcome by email, and the request status can be followed in My Cases.

Do not add weekends or U.S. holidays as though they were business days. A September request can collide with program review even if submitted before the calendar date.

If the transcript is not in English

The translation must preserve course titles, marks, grading scales, dates, duration, institutional annotations, and identity accurately.

Do not translate grades into a U.S. GPA unless the issuing school or certified translator provides an authorized explanation. Preserve the original grading system and its context.

If no transcript can be secured

Exhaust the authentic document routes first: school Entity Portal upload, official student copy, and eligible last-resort Certification transfer.

If no medical school transcript can be secured, indicate this on MyERAS Additional Documents before certification. ECFMG supplies a neutral placeholder.

The placeholder says: “This document is provided in lieu of the applicant’s medical school transcript. Please contact the applicant with any questions you may have.”

It makes the document slot appear complete for a program that requires a transcript before review. It does not provide course, hour, or grade evidence and does not guarantee that the program will consider the application complete under its own policy.

Transfer students: the governing question

ECFMG considers an applicant who attended more than one medical school to earn the degree a transfer student. Transcripts from every attended medical school should be submitted.

The submission path depends on whether the medical school of graduation participates in the MyIntealth Entity Portal—not whether the earlier transfer school participates.

The transfer-student email exception is narrow

When a participating graduation school cannot submit the earlier school's record because of institutional policy, ECFMG permits a transfer transcript PDF to be sent to the dedicated transfer-student address described on its current page.

If the applicant has a copy, the applicant may send it. If not, the transfer school's Dean's office should send it directly.

ECFMG states that the address is for transfer-student transcripts only, does not accept third-party services such as Parchment through that route, and will not process other supporting documents sent there.

Use the current ECFMG page to obtain the address; do not copy an email from an old forum.

Processing, quality control, and discrepancies

ECFMG checks the medical school transcript for clarity and correct document-slot placement. It may contact the applicant about discrepancies through the MyERAS Message Center.

ECFMG does not expedite MSPE or transcript processing. A clean document submitted early is the only reliable timing control.

A five-day processing window is not a guarantee that an illegible, incomplete, mistranslated, or mismatched record will become available without correction.

The September 11 planning date

September 11 is a recommendation designed around ordinary processing. It is not a safe target for a school that still needs weeks to issue the record or for a document needing certified translation.

For a last-resort transfer, plan around eight business days rather than five. Add a correction buffer.

Automatic residency delivery

The medical school transcript does not need program-by-program assignment for residency. It is automatically sent to all programs receiving an application.

MyERAS shows “No Available Action” in Additional Documents. That status is normal for an automatically delivered residency transcript.

After applying, MyERAS can display the date and time each document became available to a program. It does not reveal whether a program opened or read it.

Fellowship applicants use different EFDO assignment rules; do not follow a fellowship tutorial for residency.

Repeat applicants and replacement

Eligible ERAS 2026 participants may reuse the stored medical school transcript for 2027. ECFMG says it will upload within five business days after the 2027 Token is registered.

If grades, degree status, or other transcript content changed, obtain and submit a modified school-issued record through the correct owner route. ECFMG processes the new document and replaces the old version.

Do not edit the stored PDF personally. The updated record must remain source documentation issued by the school.

Transcript consistency audit

A difference may have a legitimate explanation, such as translated name order or degree date versus diploma issue date. Resolve unexplained conflicts with the issuing organization before submission.

Do not change one document to mimic another. Correct each record through its owner.
